Prenatal Case Management

 

 

Quick Facts:

Prenatal Case Management offers one on one pregnancy counseling, education and care coordination with postpartum home visits.

What is Case Management?

Case Management is a service offered to pregnant women on Medicaid who might have a potential health risk of mother or baby. Examples of a risk are:

  1. 17 years or younger
  2. smoking
  3. if you have two pregnancies less than a year apart
  4. history of premature delivery

Our goal is to help you have a healthy pregnancy and baby by:

  1. Education on pregnancy
    • Stages of pregnancy
    • What to expect
    • How your baby is developing
  2. Ways to promote a healthy pregnancy
    • Early prenatal care
    • Eating right
    • What can cause harm to your unborn baby
  3. Refer you to other services that can help with your social, financial, and educational needs.

How often do I see you?

Usually I'll see you once a month. We try to schedule these the same day of your WIC appointment.
