Prenatal Case Management offers one on one pregnancy counseling, education and care coordination with postpartum home visits.
What is Case Management?
Case Management is a service offered to pregnant women on Medicaid who might have a potential health risk of mother or baby. Examples of a risk are:
- 17 years or younger
- smoking
- if you have two pregnancies less than a year apart
- history of premature delivery
Our goal is to help you have a healthy pregnancy and baby by:
- Education on pregnancy
- Stages of pregnancy
- What to expect
- How your baby is developing
- Ways to promote a healthy pregnancy
- Early prenatal care
- Eating right
- What can cause harm to your unborn baby
- Refer you to other services that can help with your social, financial, and educational needs.
How often do I see you?
Usually I'll see you once a month. We try to schedule these the same day of your WIC appointment.
