Birth and death certificates for Newton County are maintained in this office, however, any person born in Missouri may receive a copy of their birth certificate through this office. To receive a copy of a birth or death certificate, simply come to the health department Monday thru Friday (excluding holidays ) between 8:00 a.m. and 4:00 p.m., fill out the application form, then return it to our office, or mail with the correct fee to PO Box 447/812 W. Harmony, Neosho, MO 64850..

Birth certificates are $15 each and can be obtained thru our office with cash, check, or money order. Please do not send cash in the mail. Birth certificates are available for births occurring from 1920 to present. Call 417-451-3743 for an application form or click here .

Death certificates cost $13 for the first copy, and $10 for each subsequent copy for the same person in the same visit. Death certificates are available from NCHD for deaths occurring from 1980 to present. Call 417-451-3743 for an application form or click here.
